Please be advised that Oki Data Americas, Inc., has identified a potential problem with the model OKIFAX® 5780, OKIFAX® 5980 and OKIFAX® 5980DL facsimile machines. A shield plate on these units can dislodge and cause a ground fault failure which could present a shock hazard; each of which was shipped from our facilities after October, 2003.

If you own one of the above referenced models, please turn it off and unplug it at the wall outlet.

To determine whether or not your machine is affected, after you have turned it off and unplugged it.

We will coordinate an on-site visit to inspect your fax machine, making every effort to accommodate your schedule and minimize the product's down time. If the machine passes inspection, you will be able to continue normal operation. If a problem is found, the unit will be removed and replaced by the next business day. Please allow up to two days for inspection from the time you contact either your OKI dealer or OKI Data directly. Even if your machine falls within the required serial number range and ship date time period but appears to be operating normally, you must follow the steps mentioned above to schedule an inspection. Please do not attempt to inspect the machine yourself. The procedure can only be performed by an authorized OKI technician. Also, affected machines cannot be repaired; each must be returned to the factory for proper disposal and replaced with a new unit.
